How Do I....
Register As A Patient?

All persons who request to join our practice list must live in our catchment area.

Once accepted, you will be offered an appointment for a health check with one of our nurses. Please note that you will not be formally registered with this practice until the new patient health check is undertaken.

New babies must be registered with the practice as soon as possible after leaving hospital.

All patients are now registered with the practice and not with a specific doctor, but patients may express a preference to see a particular doctor when making appointments. However, if the preferred doctor is unavailable you will be offered an appointment with another doctor. If a patient insists on waiting to see a specific doctor, this will depend on that doctor's availability and could result in the patient having to wait longer than the 48 hours anticipated in the NHS Plan.

How Do I....
Obtain Repeat Prescriptions?

Please bring or send your computer-printed counterfoil and allow two working days for processing. Telephone requests are not usually accepted. If you require your prescription to be posted back to you, please enclose an SAE with your request. Allow time for possible postal delays. You may collect your prescription from the surgery any time after 10.30am.

Medical Certificates

A doctor's medical certificate is not required for the first six days of illness. If you are off work for less than six days you should fill in a self-certification form, available from the DSS. If you feel you must have a doctor's note for less than six days, this can be provided but it will be a private one and you will be charged a fee.
